Job description

We have an urgent need for a FT PT to service our locations in Bradenton Beach to St Petersburg Florida.
This
is a salaried position but must be willing to travel during the workday to see patients at home in Manatee and Pasco County.

We are offering retention bonuses at 6months and 1 year. But must be there for the time period specified.

Reports to: Director of Nursing Job

Summary: Professional member of patient's treatment team who evaluates, assesses, and delivers services according to a written plan of treatment

established by a physician. Provides supervision for all services rendered by a physical therapy assistant. Evaluations are performed by

peers.

Job Responsibilities: Duties of the registered physical therapist include, but may not be limited to:

1. Assist physician in evaluating patient's level of physical functioning and need for physical therapy services

2. Discuss evaluation with physician to help establish a plan of

treatment to assist patient to meet maximum rehabilitation potential

3. Carry out prescribed treatments; and/or supervise PTA in carrying out PT programs

4. Establish a home program to be followed by the patient/significant other

5. Advises and consults with the family and other agency personnel and instruct patient/significant other in home programs

6. Observe and report to physician pertinent changes in the patient's

level of functioning. Communicate changes to other team members involved

in the patient's care.

7. Prepares clinical progress notes and maintain appropriate documentation of all services as per agency policy

8. Conduct self in professional manner at all times and in all situations

9. Provide agency with all required evidence, including copy of license/registration, to allow agency to verify experience

10. Accept only those assignments for which he/she is qualified

11. Participates in in-service programs as per agency requirement

12. Comply with all state, federal, local and agency rules, regulations, policies, and procedures

Job requirements

Qualifications:

Minimum: Bachelor's degree from an accredited PT program approved by the council on Medical Education of the American Medical Association

Licensed/registered in the State of the HHA as a physical therapist Minimum of two years home health experience as a physical therapist and

one year supervisory experience.
